대학병원 흉부외과 펠로우 박태수는 심장 이식을 기다리는 어머니를 부양하기 위해 오늘도 미친 듯이 일한다. 남들이 1, 2년 하는 펠로우를 4년이나 하고 있는 태수의 생사여탈권은 교수 최석한이 쥐고 있다. 태산병원 흉부외과에서 일하지만 타 대학교 출신인 두 사람은 동료들의 무시도 아랑곳없이 사람을 살리기 위해 매일 전쟁 같은 하루를 보낸다. 한편 태산병원 이사장의 딸이자 능력 있는 흉부외과의 수연은 아버지의 부름을 받고 미국 생활을 정리한 후 병원에 돌아온다. 흉부외과에 대한 자부심이 강한 그는 태산병원 흉부외과를 세계 유명 병원에 걸맞은 곳으로 키우려는 목표를 세운다. 하지만 이들이 목표를 이루려면 사람을 살리는 것 말고도 많은 것들을, 특히 깨끗하지 않은 일들도 해야만 한다.
